NOTE:

Holland America Line reserves the right to add a fuel surcharge of up to $9 per guest per day should the NYMEX oil price exceed $70 per barrel.
The operator of the private jet charter reserves the right to add a fuel surcharge 30 days prior to departure should their system wide average aviation fuel price exceed the contracted amount. Should this occur, each passenger will be billed for a proportionate share of the surcharge.
Cruise & Tour reserves the right to replace the private jet charter with scheduled airline service from either Milwaukee or Chicago should the aircraft be filled to less than 80% of capacity. Should air-inclusive reservations exceed the private jet aircraft capacity, guests booked after the aircraft is filled will be waitlisted for the private jet and accommodated on scheduled airline service from either Milwaukee or Chicago. In either circumstance, if scheduled flights from Chicago are chosen by Cruise & Tour, appropriate ground transportation between Milwaukee and Chicago will be provided at no additional charge.
